Implementing a Web-Based Cancer Survivorship Care Portal

Abstract

Poster presented June 4, 2026, at the Nursing Knowledge Big Data Science Conference held in Minneapolis.

Learning Objectives:

  1. Recognize how web-based survivorship education can be implemented in oncology practice. 
  2. Identify workflow considerations when integrating digital survivorship resources into routine care.
  3. Apply patient-reported outcome data to evaluate a technology-supported quality improvement initiative.

Introduction / Problem: Cancer survivorship care remains inconsistently delivered despite recommendations supporting survivorship care planning and coordination across oncology and primary care. Survivors frequently report unmet informational needs, and workflow constraints can limit consistent survivorship education delivery.  Digital survivorship tools may offer scalable access to education and reinforce engagement with the care team.
